Dans la terminologie Bethesda 2001, les frottis doivent \u00eatre class\u00e9s dans la cat\u00e9gorie &#8220;satisfaisant pour l&#8217;\u00e9valuation&#8221; ou &#8220;non satisfaisant pour l&#8217;\u00e9valuation&#8221;. La cat\u00e9gorie &#8220;satisfaisant mais limit\u00e9 par&#8221; des pr\u00e9c\u00e9dentes classifications Bethesda a \u00e9t\u00e9 \u00e9limin\u00e9e. La pr\u00e9sence ou l&#8217;absence d&#8217;\u00e9l\u00e9ments endocervicaux\/de la zone de transformation et d&#8217;autres facteurs de qualit\u00e9 peut \u00eatre incluse comme composant descriptif de la cat\u00e9gorie &#8220;Satisfaisant&#8221;, mais pas pour &#8220;limiter&#8221; la qualit\u00e9. La pr\u00e9sence d&#8217;un \u00e9l\u00e9ment de la zone de transformation doit \u00eatre justifi\u00e9e par l&#8217;observation d&#8217;au moins 10 cellules endocervicales ou malpighiennes m\u00e9taplasiques bien conserv\u00e9es. Si une l\u00e9sion de haut grade ou un cancer est observ\u00e9(e), il n&#8217;est pas n\u00e9cessaire de signaler la pr\u00e9sence\/absence de cet \u00e9l\u00e9ment.<\/p>\n<p class=\"subhead\"><a name=\"satisfactory\"><\/a>SATISFAISANT POUR L&#8217;\u00c9VALUATION<\/p>\n<p>En g\u00e9n\u00e9ral, un \u00e9chantillon doit contenir au minimum 5 000 cellules malpighiennes bien visualis\u00e9es\/conserv\u00e9es pour \u00eatre consid\u00e9r\u00e9 comme satisfaisant. Les \u00e9l\u00e9ments suivants doivent cependant \u00eatre pris en compte :<\/p>\n<ul class=\"normal\">\n<li>Des crit\u00e8res objectifs stricts ne peuvent pas \u00eatre appliqu\u00e9s \u00e0 tous les cas. Par exemple, les lames avec regroupement cellulaire, atrophie ou cytolyse sont techniquement complexes \u00e0 compter. Les laboratoires doivent donc appliquer leur jugement professionnel et employer un processus d&#8217;examen hi\u00e9rarchique pour \u00e9valuer ces lames.<\/li>\n<li>L&#8217;\u00e9valuation de la cellularit\u00e9 ne n\u00e9cessite pas toujours un comptage. Si le cytotechnologiste pense que l&#8217;\u00e9chantillon contient mois de 5 000 cellules malpighiennes bien visualis\u00e9es\/conserv\u00e9es, il peut avoir recours \u00e0 un comptage sur le mod\u00e8le suivant.<\/li>\n<\/ul>\n<p>Tous les \u00e9chantillons contenant des cellules anormales sont par d\u00e9finition satisfaisants pour l&#8217;\u00e9valuation. S&#8217;il s&#8217;av\u00e8re n\u00e9cessaire de compter les cellules malpighiennes dans une lame ThinPrep : 1. un minimum de 10 champs doit \u00eatre compt\u00e9 dans un diam\u00e8tre qui inclut le centre de la pr\u00e9paration. La surface du champ peut \u00eatre d\u00e9termin\u00e9e par la formule de calcul de la surface d&#8217;un cercle [pi x R\u00b2].<\/p>\n<p>Pour les lames trait\u00e9es avec le syst\u00e8me d&#8217;imagerie ThinPrep<sup>\u00ae<\/sup>, la qualit\u00e9 doit \u00eatre d\u00e9termin\u00e9e selon la proc\u00e9dure d\u00e9crite dans le manuel de l&#8217;op\u00e9rateur.<\/p>\n<p>Le syst\u00e8me Bethesda 2001 indique que des informations suppl\u00e9mentaires relatives \u00e0 la signification des facteurs de qualit\u00e9 et des implications en termes de suivi de la patiente peuvent \u00e9ventuellement \u00eatre fournies dans une note. Le traitement et l&#8217;\u00e9valuation de pr\u00e9l\u00e8vements non satisfaisants sont tr\u00e8s fastidieux. En effet, m\u00eame si ces pr\u00e9l\u00e8vements ne peuvent pas exclure de l\u00e9sion \u00e9pith\u00e9liale, certaines informations utiles (pr\u00e9sence d&#8217;organismes infectieux, etc.) peuvent \u00eatre fournies pour faciliter la prise en charge directe de la patiente. Voici les termes indiqu\u00e9s par le syst\u00e8me Bethesda 2001 pour clarifier ces \u00e9l\u00e9ments :<\/p>\n<p><strong>A) Frottis rejet\u00e9 : <\/strong><br \/>\n\u00c9chantillon rejet\u00e9 (non trait\u00e9) car ____ (\u00e9chantillon non \u00e9tiquet\u00e9, lame cass\u00e9e, etc.)<\/p>\n<p><strong> B)<a name=\"unsatisfactory\"><\/a> Frottis non satisfaisant int\u00e9gralement \u00e9valu\u00e9 : <\/strong><br \/>\n\u00c9chantillon trait\u00e9 et examin\u00e9, mais non satisfaisant pour \u00e9valuation des anomalies \u00e9pith\u00e9liales en raison de ____ (\u00e9pith\u00e9lium malpighien insuffisant d\u00fb \u00e0 la pr\u00e9sence de sang, etc.). Commentaires\/recommandations suppl\u00e9mentaires, si n\u00e9cessaire .<\/p>\n<p><strong>FACTEURS MASQUANTS <\/strong><br \/>\nLes \u00e9chantillons avec &gt;75 % de cellules masqu\u00e9es doivent \u00eatre class\u00e9s dans la cat\u00e9gorie &#8220;non satisfaisant&#8221; (supposant l&#8217;absence de cellules anormales). Si entre 50 et 75 % de cellules sont masqu\u00e9es, le terme &#8220;satisfaisant&#8221; doit \u00eatre suivi d&#8217;un commentaire indiquant que l&#8217;\u00e9chantillon est partiellement masqu\u00e9.<\/p>\n<p>Les microphotographies de cet atlas pr\u00e9sentent un large \u00e9ventail d&#8217;\u00e9chantillons cellulaires contenant du mat\u00e9riel malpighien et glandulaire ad\u00e9quat. Nous avons \u00e9galement inclus des microphotographies illustrant des \u00e9chantillons qui contiennent le nombre minimum de cellules, ainsi que des exemples d&#8217;\u00e9l\u00e9ments de la zone de En cas d&#8217;accumulation de d\u00e9bris inflammatoires, de sang ou de mucus sur le col, cette accumulation appara\u00eet sur la lame si elle n&#8217;est pas retir\u00e9e au moment du pr\u00e9l\u00e8vement.
